Under the direction of the Executive Director, Clinical Nurse Manager, or Clinical Nurse Director, the WelbeHealth Assistant Clinical Nurse Manager II is accountable for organizing and directing the day-to-day RN Case Manager program. This role may also involve overseeing clinic staff and coordinating clinic flow to ensure effective WelbeHealth clinical services are provided, and quality of care standards are met.

On the day-to-day, you will:
Provide leadership, oversight, aid, guidance, and performance management to RNs (and potentially to LVNs and Mas), ensuring they all have role clarity and the training required to perform their functions at the top of their license
Oversee the clinical needs in the center to ensure chronic and acute participant needs are addressed properly, and liaise with the interdisciplinary team (IDT) on smooth transitions of care to meet post-discharge needs
Develop workforce plans and ensure appropriate staffing, recognizing diverse staff experience levels and unique participant needs
Effectively intercede with participants and their families to resolve concerns related to the delivery of health care services
Manage direct reports including hiring, training, supervising, and mentoring, possibly having direct reports across multiple sites
Participate in any required Quality Improvement activities, staff training, and meetings
Job requirements include:
Graduate of an accredited school of nursing with unencumbered RN license with which you practice required; bachelor’s degree (BSN) preferred
One (1) year of supervisory experience with demonstrated aptitude to mentor and develop team members preferred
Minimum of two (2) years of case management, care coordination, or related specialty experience
Excellent leadership, organizational, and communication skills in settings with seniors, their families, and IDT members
Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects
Highly motivated, inspirational leader who can engage, support, and remove barriers for the team to ensure the team consistently meet established goals
