We automate this work with AI, creating a fundamentally different cost structure compared to incumbents. This lets us rapidly take market share by serving the 30% of patients who go untreated today.
We have built one of the best AI teams in the world (from Scale, Palantir, Citadel, Jane Street) and paired them with healthcare veterans and ex-MBB strategists to build a new type of healthcare company: one that delivers care at the speed of an AI company.
We are starting with home health, then expanding to all delivered care. Our mission is "any care, any where." Fundamentally, our incentives are aligned with America. Every dollar in revenue we make is two dollars taken out of the healthcare system. If we succeed, so does US healthcare.
This is a Special Projects Lead seat. Our SPLs take ambiguous, high-stakes problems and own them end to end. This one owns the thing our partnerships are sold on: our strategic clinical programs.
We are building a focused menu of clinical programs (orthopedics, cardiovascular/heart failure, and oncology) that let us go to health systems with something specific and provable rather than generic home health offerings. Those programs are what our partners buy, and they are how we win at-risk and value-based arrangements as physicians move to risk.
We are actively building our clinical program menu. Ortho is live and being refined based on early learnings. Cardiovascular is in development. Oncology is next. This role will be the first owner of the full lifecycle: designing each program, executing delivery, and generating the outcomes evidence that scales them.
You will be that person. You will own the program menu end to end: design what each program actually is, stand it up in market, deliver on what Partnerships sells, and produce the outcome data that turns one pilot into a pipeline. Think of it as a blend of:
Clinical program design (decide what a program actually consists of: which patients, which visit patterns, which clinicians, which protocols, and what we promise)
Delivery ownership (when Partnerships signs a health system, you are accountable for executing it and for the numbers we report back)
Evidence generation (build the reporting that proves our outcomes, because the data is what unlocks the next partnership and the next market)
You will work directly with Partnerships, Clinical, Operations, and our market GMs, and you will be in the room with hospital clinical leadership. Like all SPL roles, this is a track: it is designed for people who want to grow into owning a function, a P&L, or a GM-style seat.

1. Own the clinical program menu
Own our core home health programs end to end: orthopedics, cardiovascular/heart failure, and oncology (others under consideration)
Define what each program actually is: target patients, visit cadence, clinical protocols, staffing model, telehealth component, reporting, and what we are willing to promise a partner
Keep the menu disciplined. A focused set of programs we deliver exceptionally beats a long list we deliver adequately
Coordinate with our Complex Care offering: a high-acuity patient list delivered with excellent metrics and home health, used to establish fair market value
2. Deliver on what Partnerships sells
When we sign a health system, you own execution. You are accountable for the program actually running the way we said it would
Build each partnership's bespoke program and live reporting, working with our clinical leaders in market
Own the numbers we report back to partners, including in at-risk and pilot structures where our payment depends on outcomes
3. Prove it worked, then scale it
Build the outcome reporting that turns a single pilot into a referral pipeline. Early, credible data is the entire unlock
Take a program that worked in one hospital and make it repeatable in the next one, and the next market
Partner with Growth and Operations to stand programs up as we launch new regions
4. Define the metrics that prove the model
Own the KPIs for each program: re-hospitalization, time to start of care, patient outcomes, margin, referral conversion
Know the perverse incentives your metrics create and design around them. If a program's payment depends on an outcome, you need to know exactly how the baseline was set and where it can be gamed
Make each program's performance legible to partners, to leadership, and to our own market teams
A clinical background. RN, therapist (PT/OT/SLP), NP, PA, MD, or a clinical operations leader who has run clinical delivery. You have to be credible with clinicians and with hospital clinical leadership, because you will be in those rooms constantly
3-5 years owning complex, ambiguous projects end to end, where you had many stakeholders and no playbook, and you delivered. You can articulate the load-bearing decisions and why they mattered
Analytical rigor. Comfort with data analysis (SQL, Excel modeling, Claude Code) to build a business case, set a baseline, and defend a number to a partner who is motivated to argue with it. This is not optional. A clinician who cannot do the math will not succeed here
Exceptional written and verbal communication. You can synthesize a complex clinical and financial picture into something crisp, and build a deck a founder would put in front of a health system executive
Track record of owning outcomes, not just output. Work that demonstrably moved patient outcomes, volume, cost, or revenue
Operator mindset: you take a vague ask and return something excellent without needing it fully specified. You decompose messy problems, prioritize ruthlessly, and treat a program rollout like a workflow with owners, deadlines, and failure modes
Accountable end to end: you unblock yourself, escalate when necessary, and iterate quickly
Clinically grounded, commercially sharp: you can hold a real conversation about heart failure readmissions and about the margin on the contract that pays for it
Comfort with ambiguity: you can operate zero to one. Our programs are early and rough, and that is the job
Low ego, high standards: this is a startup. You get scrappy, you sweat the details, and you hold the line on quality and on what we are willing to promise
Experience building or running a service line (ortho, cardiac, oncology) at a health system, or standing up a clinical program at a provider organization
Home health, post-acute, or hospital-at-home experience
Exposure to value-based care, at-risk contracting, bundled payments, or CMS models
Experience with health system partnerships or JVs
Background in management consultant
Comfort with telehealth-enabled care models
